FOG custom report

Is there a video or instructions on how to use the report template? Or is there a way to see the reports that are being used already to tweek them. I only want a simple .csv file with host name, serial number, and date created.

I realised I did not put my versions. FOG is 1.2 and my OS is Ubuntu 13.10

I’ve never used the reporting functionality in FOG…
I’d personally just fire up an SQL client, connect to FOG, do the query for the data I want, dump that into a Google Doc, and format it up nice and pretty.
