On which operating system did you install FOG to? Which version of FOG did you install was it 0.32 as reflected in the picture? Do you have a firewall or other blocking software active such as antivirus firewall, or ufw?

This is a known issue with FOG 0.32.
Open up the tasks.confirm.include.php file and replace all instances of &$tmp to $tmp.
