Create New Image links to a blank page

Just installed FOG 1.2.0 on Ubuntu 12.04. I followed the directions for FOG on an isolated network, and have been able to PXE boot, but can’t create a new image from the management console. When I click on “Create New Image” it brings me to a blank page that simply says “Add new image definition” in the upper left corner and there are no fields to type in.
