Task Management blank on Upload

I’m using Ubuntu Server 13.04, I’ve got an external DHCP server that I can confirm works with PXEBoot, and I can inventory machines without a problem. What I’m running into is that when I click Upload on one of my hosts to upload the machine’s image to the server, I get a blank Task Management screen. The same is true for Deploy and Advanced Options.
I’ve already edited the suggested file on forums regarding this issue and changed &$tmp to just $tmp, but it hasn’t solved the problem for me, and I’d really like to get this going.
Any ideas? Thanks.

Does the task actually perform?
What version of FOG are you using?
Can you give us, right after you’ve created an upload/deploy/advanced task, a snippet of your error.log file located in /var/log/apache2/error.log?
