Host Import Fail

I am having an trouble figuring out what I am doing wrong when I import hosts. Every time I try and import a CSV I get an error saying “Invalid number of cells”. The error only shows 11 rows, but I have about 400 rows. Here is a quick look at what my first 6 rows look like. From the wiki it sounds like the other info (Description, OSID, etc) is not required.
