Add button not working for 'Create New Image'

I just setup FOG server 1.5.1 and everything looks good. I’m able to PXE boot a machine to access the server. I want to take an image of the remote machine, yet, I can’t add an image.

I fill out the ‘Image Name’, "Operating System’ and other fields, however, when hitting the ‘Add’ button, nothing happens.

I am, however, able to create new users, groups and hosts.

Any help would be greatly appreciated.

Ok, I figured it out. Turns out the mysql db wasn’t properly setup. Deleting the old files, reinstall and re-setting up did the trick!
